How to Login

Double-click the Online Document System icon on the desktop or go to http://onlineforms.gehrhsd.net/forms/signin.asp.You then log in from the Welcome Window that displays. The Internet Explorer application will start and connect you to the online system.

The sign on screen requires you to provide you user ID and you password to access the program.

Your user ID should be the same as your Web mail account. Your password is the last 4 digits of your social security number.

Welcome Screen

The welcome screen is split into 3 parts, the navigation menu on the left, the available forms at the top green area, and a list of the forms you have completed at the bottom in the green area.
